COVERED SERVICES

Well Child Office Visit
Following the America Academy of Pediatrics Standard of Care Guidelines for Primary Care Visits, the below is recognized as the typical well child office visit schedule:
·         Infancy - 1, 2, 4, 6, 9 & 12 Months of Age
·         Early Childhood - 12, 15, 18, 24 Months & 3 Years of Age
·         Middle Childhood - 4, 5, 6, 8, & 10 Years of Age
·         Adolescence - Every Year from 12 - 18 Years
 
Sick Child/Chronic Care Office Visit
Primary Care Visits for children outside of the well child primary care office visit schedule. Maximum 2 visits per year. May be extended for Chronic Care Case Management with Physician’s planned directive.
 
Supplemental Supplies/Durable Medical Equipment
Items such as Nebulizers, Oxygen, or other small Durable Medical Equipment that is locally held in inventory at a medical clinic. (Final list of covered items To Be determined.)
 
Immunizations/Vaccines
Coordinated through the Vaccines for Children Fund.
Vaccines prevent disease in the people who receive them and protect those who come into contact with unvaccinated individuals. The current Recommended Childhood and Adolescent Immunization Schedule as defined by the Department of Health and Human Services Centers for Disease Control and Prevention will be followed.
Prescription assistance
Prescription assistance provides necessary medications to pediatric patients who cannot afford them. Pharmaceutical prescription assistance through CATCH and networking with existing programs in the community will be utilized to meet the short and long-term prescription assistance need.
